Submit an online report

Purpose of Form:

This form allows any member of the Juilliard community to report conduct that is prohibited under the Non-Discrimination and Sex-Based Misconduct Policies. This includes, but is not limited to, incidents of discrimination, harassment, and sex-based misconduct. The report will be sent to the Title IX Coordinator for review and action. Please note that you do not have to use this Form.  You may report allegations of prohibited conduct verbally, in-person, by email, text, or in any other manner that provides notice to the School.

Reports involving the Tianjin Juilliard School, located in Tianjin, China, will be shared with the Senior Coordinator at the Tianjin Juilliard School. This is a separate reporting process from reporting to the Title IX Coordinator, and is specifically for incidents that occur at the Tianjin Juilliard School.

Important information when filling out the webform:

  • Please log out of your Juilliard account if you would like to submit an anonymous report. It is important to note that anonymous reporting may reduce the School's ability to respond or pursue appropriate action depending on the level of information available about the incident or the individuals involved. Confidential reports can be made to:
    1. Health Services (students only)
    2. Counseling Services (students only)
    3. Employee Assistance Program (employees only)
       
  • The Office of Non-Discrimination and Title IX will follow up with you to discuss the incident and the next steps. You are not required to speak with the Office of Non-Discrimination and Title IX for making a report.
  • Please remember that the Office of Non-Discrimination and Title IX is NOT a confidential resource. Our Office ensures the privacy of information to the greatest extent possible and only shares information on a "need to know" basis to assist with response.

After Submission:

Once you click submit, your report will be transmitted directly to the Office of Non-Discrimination and Title IX team members. Upon receipt of this form, a member of the team (most likely the Title IX Coordinator) will reach out to you as promptly as possible to discuss all of your rights and options under the Policies and Federal and State law, including the availability of supportive measures, on- and off-campus resources, and options for how to proceed with the report.
